Request for Exclusion Form

To exclude yourself from the Settlement Classes, receive no benefits from the Settlement, and retain all your rights, you must return your Request for Exclusion Form by September 20, 2019. Please read this form carefully and follow the instructions below.

You may create your Request for Exclusion online by clicking “File an Exclusion” below, completing the required information, and printing the form. Then, you must sign the form and upload it by September 20, 2019. You may also print out and mail the form, available on the Important Documents page. If you are represented by Counsel, the Request for Exclusion Form must be signed by both the individual Member of the Settlement Class and Counsel. If mailed, your Request for Exclusion must be received by September 20, 2019.

Your Request for Exclusion is not complete until you sign and return it to the Settlement Administrator. You may submit your Request for Exclusion Form via mail, email, or upload it below.
